  • Claim Form completed by policyholder & the attending Doctor (mandatory)
  • Original Bills & Receipt for medical expenses (mandatory)
  • Police Report (applicable for Motor accident) (mandatory)
  • Original Receipt for Ambulance fees (if applicable) (optional)

Hospitalisation Claim: Documents Needed
